Breast Ultrasound

Loomis-Alvarez Video Series correlating structure, function, and clinical applications to facilitate medical student understanding of complex anatomical and physiological concepts. In this episode, we present a breast ultrasound with live imaging correlated with mammography. Breast cancer is the most common malignancy in women and the 5th leading cause of cancer death worldwide. Therefore, screening and early detection are critical. While mammography is the primary screening modality, ultrasound can be complementary and, in some cases, preferable for younger women and those with dense breast tissue. It is particularly useful in distinguishing cysts from solid masses. Ultrasound is also useful as a guide to biopsying and excising breast tumors. Ultrasound is portable, avoids ionizing radiation with its cancer risk, and is easily performed in an office setting. By following the position of the probe on the breast, juxtaposed with the ultrasound image, students and residents can improve their understanding of the structures of the breast, chest wall, and axilla, and they relate to the physical examination and diagnostic imaging.
Data: Integrating Domains of Clinical Reasoning into Medical Students' First Weeks of Education Improves Understanding of Complex Neuroanatomy

A quarter of all medical errors are due to faulty clinical reasoning. Such reasoning involves analytical (deductive) and non-analytical (recall) processes. The non-analytical is taught extensively in medical school, and it dominates medical students' time as they review question banks and lecture notes, watch videos online, and memorize flashcards, algorithms, and illness scripts. However, the step-by-step deductive process of analytical reasoning is taught much less often. When it is, the diverse levels of innate reasoning ability among students often lead to significant learning disparity. To address this deficiency, a pilot module on cranial nerve anatomy was developed to enhance analytical reasoning skills. It was hypothesized that this module would not only introduce an essential medical skill but also improve overall student understanding of the subject and reduce learning disparities among students. A comparative study was conducted between this novel module and a control didactic module, employing pre- and post-testing measures. Results indicated a 26% improvement in average scores following the analytical module, whereas the control module showed no significant improvement. In addition, there was less disparity following the analytical module with 75% of students achieving improved results, compared to 41% following the control. Analytical reasoning was successfully introduced early in first-year medical students' education, facilitating the learning of complex neuroanatomy and reducing learning disparity between students. Future research should evaluate the long-term retention of these benefits, their impact on analytical reasoning skills during the clerkship years, and their effects on performance in level two and three licensing examinations.
An evidenced based evaluation on the outcome of osteopathic manipulative treatment on postoperative coronary artery bypass and sternotomy patients

The recent COVID-19 pandemic caused a widespread strain on healthcare systems around the world, including the United States. With limited resources, such as healthcare staff and available beds in intensive care units (ICU), consideration must be placed on effective solutions. The goal of our study is to find evidence supporting the effectiveness of osteopathic manipulative treatment (OMT) on ICU length of stay for post-operative patients that have undergone coronary artery bypass graft (CABG) surgery via sternotomy. We believe our findings will increase awareness of application of the beneficial effects of OMT in critical care patients.

In this episode of the Loomis-Alvarez Medical Video Series, we present a systematic approach for a critical interpretation of compensatory acid-base disturbances in Arterial Blood Gases (ABGs). In this video, we will decipher compensatory acid-base disturbances in arterial blood gases, a skill that helps diagnose and manage critical conditions. We will follow a systematic approach, first identifying if there is acidemia or alkalemia by looking at the pH. Then, we will determine the primary culprit – is it a respiratory issue reflected in the pCO2, or a metabolic problem evident in the HCO3- level? The fun part comes next: analyzing the compensatory response. Did the body attempt to adjust the pH with the respiratory system (by altering CO2) or with the metabolic system (through HCO3- changes)?
